By incorporating these steps into your healthcare routine, you help us advocate for your wellness and protect your access to vital resources.
Formally selecting your PCP is the foundation of excellent healthcare. We ask that you formally select Sarah Newhouse as your Primary Care Provider. She will act as your main health advocate, helping you coordinate all aspects of your physical and preventative wellness.
Proactive health management is key to preventing major medical issues before they start. By scheduling a regular quarterly check-in with our team, we can stay ahead of any changes in your health and monitor your well-being in real-time.
At every appointment, please bring a complete list of everything you take. This includes:
Prescriptions from any doctor
Over-the-counter (OTC) medicines
Traditional herbs, remedies, and supplements
Having this complete list allows us to keep your medical records entirely accurate.
If you receive care from doctors, specialists, or therapists outside the JBCI Tribal Clinic, please sign a Release of Information (ROI) form. This gives us permission to securely share updates with your outside providers, ensuring your entire care team is working together on a single, unified care plan.
